In one randomized, controlled trial, patients with type 2 diabetes who underwent laparoscopic flexible gastric banding got a higher rate of remission of hyperglycemia than do patients who were treated medically.15 However, patients in that study acquired relatively mild diabetes of short duration , and diabetes remission was predicted based on the amount of postoperative weight loss. A meta-analysis16 of studies on numerous bariatric procedures involving patients with type 2 diabetes showed a standard price of remission of hyperglycemia of 78 percent among the many procedures.Only a small number of brain-donation programs are linked to longitudinal, population-based research of aging.9 Applications that have included persons in the oldest age groups consistently show that these persons possess mixed neuropathological features10,11 and that those who die in their 80s and 90s may have pathological features of Alzheimer’s disease without a medical diagnosis of dementia during life.12 Current diagnostic requirements that seek to define Alzheimer’s disease, vascular dementia, and Lewy body dementia may not apply as well to the oldest old as to the younger old, in whom these were generated.
